Предпосылки и цель исследования. Проведение системного тромболизиса альтеплазой одобрено при остром ишемическом инсульте, но его использование ограничивается многочисленными противопоказаниями и предостережениями, связанными с критериями отбора в испытаниях или экспертными заключениями. Изучили исходы, зарегистрированные в архиве испытаний, у пациентов, получавших альтеплазу, по сравнению с пациентами, которым не проводили системного тромболизиса в соответствии с наличием или отсутствием указанных противопоказаний и предупреждений. Методы. Проанализировали оценки по модифицированной шкале Рэнкина через 90 дней, согласно распределению всех оценок с помощью метода Кокрейна-Мантеля-Хэнзеля, с внесением поправок на возраст и исходную оценку тяжести инсульта по шкале Национальных институтов здравоохранения (NIHSS) и последующим проведением логистического регрессионного анализа пропорциональных рисков для оценки отношения шансов развития предпочтительного исхода. Результаты. Использовали данные 9613 пациентов с ишемическим инсультом, из которых 2755 лицам проводили системный тромболизис. Скорректированные отношения шансов (ОШ) продемонстрировали наличие общей тенденции к развитию более благоприятного исхода через 3 месяца, связанного с проведением системного тромболизиса, по сравнению с отсутствием лечения в различных подгруппах пациентов с противопоказаниями и предостережениями: например ОШ=1,40 (95% доверительный интервал [ДИ] от 1,14 до 1,70) для пациентов в возрасте старше 80 лет (n=1805), ОШ=1,50 (95% ДИ от 1,03 до 2,18) для пациентов с наличием в анамнезе ранее перенесенного инсульта и сахарного диабета (n=672), ОШ=1,42 (95% ДИ от 1,19 до 1,70) для пациентов, получавших монотерапию антиагрегантами (n=1626), ОШ=2,20 (95% ДИ от 1,12 до 4,32) для пациентов, получавших оральные антикоагулянты при значении международного нормализованного отношения ≤1,7 (n=157), ОШ=1,50 (95% ДИ от 1,15 до 1,97) для пациентов с исходным уровнем глюкозы >180 мг/дл (n=879) и ОШ=1,57 (95% ДИ от 1,12 до 2,18) для пациентов с оценкой по шкале NIHS >22 баллов до начала лечения (n=620). Выводы. Проведенный комплексный ретроспективный анализ различных противопоказаний и предупреждений подтвердил наличие преимуществ и рисков системного тромболизиса с использованием альтеплазы в распространенных клинический ситуациях.
